NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis ; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. § 1915, permits him to do so if, among other things, he files a certified copy of his inmate trust account statement for the six month period preceding his complaint. In this case, the plaintiff did not do so. This is the fifth case that the plaintiff has filed within the past year. In his first case, Petteway v. Connecticut Department of Corrections , 23-cv-1101 (SVN), he filed a proper inmate trust account statement, and the Court promptly granted him in forma pauperis status after he did so. But in his last four cases, including this one, he has filed only a "PLRA In Forma Pauperis Status Report." The Court wishes to be clear with Mr. Petteway that a PLRA In Forma Pauperis Status Report is not sufficient. The in forma pauperis statute requires a proper trust account statement, certified as accurate by a prison official. See 28 U.S.C. § 1915(a)(2). The plaintiff is therefore directed to file with the Clerk of the Court a certified copy of his inmate trust account statement covering at least the period from October 19, 2023, to April 19, 2024.
